A.)  Identify students using quantitative/ qualitative data- identifying concerns of speech/ language, behavior, reading, writing or math (after the first 6 weeks of school)

B.)  Discuss concerns with students’ parents & document/ date when discussed.  The purpose of this data is to gather information/data about the student whether they previously had an IEP, physical limitations, diagnoses, family/ homelife etc. The teacher will inform the student’s parent that you would like to pursue some interventions.

Step 2

C.) The teacher will fill out the SIT Student Referral Profile- put into Ms. Lana’s mailbox (If you need help filling this out please ask Ms. Lana or anyone on the SIT team)

D.) Walkthrough be done by Ms. Lana, Ms. Nicoletta, or Ms. Dana

Step 3

E) If the walkthrough is not approved, tier 1 interventions will be suggested to implement for 6 weeks. After 6 weeks if these tier 1 interventions are not working then there will be a second walkthrough and consideration for further process.

F) If walkthrough is approved, there will be a referral to the SIT chairperson, Ms. Lana, who will schedule a meeting with you and the SIT team to suggest tier 2 interventions for 6 weeks.

Step 4

G.) Collect quantitative and qualitative data for the tier 2 interventions for 6 weeks.

H.). At the end of 6 weeks the teacher will attend a SIT meeting with your observations, data, & samples.

The team will decide to either continue the same interventions for another 6 weeks (with data), or change interventions for another 6 weeks (with data).

Step 5

I.). At the end of the next 6 weeks, teachers are invited to another SIT meeting.  Bring data, observations and samples.

Step 6

J) After there have been 12 weeks of Interventions, Lana will meet with the SIT team &  the following actions will be determined:

1.)  Refer for  tier 3 intervention

2.)  Recommend the same, or alternative intervention for another 6 weeks.

3.) Recommend for testing

4.) Refer for 504

Step 7

K) If recommended for testing, a consent form to have your student tested will be sent home. When the form is received by the Special Education Director, Ms. Dana, your student then has 45 school days to be tested. These days DO NOT include weekends, holidays, and days that school is not in session.

1.) Once your student has been tested, a meeting will be held to go over testing results and determine if your student qualifies for Special Education services. If it is determined your student qualifies for Special Education services, then an Individualized Education Plan (IEP) will be created.
